Pharmacy Fellowship Application Requirements

To apply to the Valley Children’s PGY2 Pediatric Pharmacy Residency Program, candidates must submit an application through ASHP PhORCAS™ (Pharmacy Online Residency Centralized Application Service). The program participates in the ASHP Resident Matching Program (Match). (Program Code: 736961). Applicants must complete all standard PhORCAS application requirements and submit the supplemental materials listed below.

1. Doctor of Pharmacy degree from an ACPE accredited school of pharmacy; or a M.S. or B.S. in Pharmacy with equivalent clinical experience.
2. Have successfully completed, or be on track to successfully complete, an ASHP-accredited PGY1 Pharmacy Residency Program prior to the start of the PGY2 residency year.
3. Hold a current California pharmacist license or meet all requirements for California pharmacist licensure and obtain licensure within 30 days of the start of the PGY2 residency program. Applicants who are not licensed as a California pharmacist by the residency start date must possess a valid California pharmacist intern license and maintain eligibility for pharmacist licensure until full pharmacist licensure is obtained.
4. Apply for ASHP Residency Matching Program (PGY2 Match Code 736961)
5. Three professional recommendations in PhORCAS (completion of standard PhORCAS reference form) - If you have completed a pediatric rotation in PGY1, it is highly recommended that one reference be from that pediatric preceptor.
6. Your letter of intent uploaded to PhORCAS should answer two questions
   1. Why you want to complete a PGY2 pediatric residency
   2. Why you want to complete a residency at Valley Children's Hospital

Once we review your application materials, we will rank candidates and offer interviews (dates to be determined). Interviews are approximately 3-4 hours in duration and generally include a hospital tour, interviews with preceptors, and a presentation on a pediatric topic.
